If you are a living individual residing in the United States and your Private Information was potentially compromised during the targeted cyberattack on Youth and Shelter's computer system that occurred in September 2023 (the "Data Incident"), including all those who received notice of the Data Incident, then you may be a Class Member. Excluded from the Settlement Class are: (a) all persons who are directors and officers of Defendant; (b) governmental entities; (c) the Judge assigned to the Action, that Judge’s immediate family, and Court staff; and (d) any other person found by a court of competent jurisdiction to be guilty under criminal law of initiating, causing, aiding or abetting the criminal activity occurrence of the Data Incident or who pleads nolo contendere to any such charge.

If you are a Class Member and wish to receive Settlement Benefits then you must submit a Claim Form by July 20, 2026.

All Settlement Class Members may claim one of two Cash Payment options. The benefits are explained in more detail below.

Cash Payment A – Documented Losses. If you incurred actual, documented out-of-pocket losses due to the Data Incident, you can get back up to $2,500.00. The losses must have occurred between September 2023, and July 20, 2026, and you cannot claim a payment for expenses that have already been reimbursed by a third party. This benefit covers out-of-pocket expenses like:

  • losses because of identity theft or fraud

  • fees for credit reports, credit monitoring, or freezing and unfreezing your credit

  • cost to replace your IDs

  • postage to contact banks by mail

You need to send proof, like receipts, to show how much you spent or lost. Your personal certifications, declarations, or affidavits do not constitute reasonable documentation to make a valid claim, but you may include that to provide clarification, context, or support for other submitted reasonable documentation showing that your expenses were because of the Data Incident.

Cash Payment B – Alternative Cash. Instead of Cash Payment A, you may claim a one-time $50.00 cash payment. You do not have to provide any proof or explanation to claim this payment.

The deadline to submit a Claim Form online is July 20, 2026. If you are mailing your Claim Form, it must be mailed with a postmark date no later than July 20, 2026.
